Financial Assistance Guidelines

All financial assistance is available to U.S. citizens only and is based on the donations ALODF receives each year.

• All applications are reviewed individually and approved at the sole discretion of ALODF.

• All decisions are final.

• Paper applications will not be accepted.

• ALODF may approve an amount different from the amount requested.

• We have a maximum of $3,000 for kidney donation and a maximum of $4,000 for liver donation.

• Applications must be submitted before applicant selection. We prefer applications before organ donation but will accept applications up to two months after surgery.

• Please review all instructions carefully and complete the applications with your transplant team wherever possible.

Required Documentation:

• Signed letter from your employer stating if you are using paid time off and/or short-term disability provided.

• 6 months of payroll documentation (If possible, please combine this into one PDF)

• Most recent income tax submission showing total income

• Letter from your surgical team indicating expected time off for recovery

• Confirmation from your social worker that the donation surgery was completed, including the date of donation.

• All receipts for requested travel expenses (required before travel assistance can be considered)

Zelle Disbursement

If your application is approved, funding is issues through Zelle. You must provide your Zelle ID so ALODF can process the payment.
